Abstract

L’approche communautaire et sociale en ergothérapie, pratique encore peu développée en France et en Europe pourrait connaître un développement important car plusieurs éléments des contextes économique, social et politique actuels sont en faveur de telles pratiques. Les bouleversements économiques amènent de la précarité, de la pauvreté et de l’instabilité, provoquant des Inégalités Sociales de Santé (ISS) et entraînant de nouveaux problèmes pour différents groupes sociaux (migrants, chômeurs, détenus, jeunes, personnes âgées, femmes, etc.). La lutte contre les ISS est une priorité de santé publique en France. L’approche communautaire est particulièrement indiquée pour atteindre et accompagner les publics fragilisés, et en ergothérapie, nous pouvons agir auprès de ce public précaire en envisageant les problématiques, en termes d’occupation. Henneke van Bruggen (2014) propose de « s’engager dans la justice occupationnelle en développant des approches communautaires fondées sur l’occupation dans lesquelles tous les individus trouvent leur place. » L’approche communautaire se base sur une méthodologie spécifique en plusieurs phases. La première concerne la communauté qui doit être identifiée et reconnue. La deuxième phase correspond au diagnostic communautaire qui met en exergue les problématiques occupationnelles et de qualité de vie de la communauté. Ensuite c’est l’étape de la planification des actions, puis de leurs réalisations et pour finir la phase de l’évaluation de l’action. Cette démarche est une démarche bottom-up, qui part de la communauté pour coconstruire les actions. L’ergothérapeute doit s’engager à favoriser le bien-être de communautés précaires, il agit comme un acteur de changement. En tant qu’agent de changement, l’ergothérapeute doit être prêt à apprendre des communautés, à connaître les communautés, leurs contextes, leurs habitudes pour agir différemment dans la pratique.

The social and community based approach in occupational therapy is a practice that is still underdeveloped in France and in Europe. Nevertheless it could be an important field of development because several elements of the current economic, social and political contexts promote such practices. Economic upheavals lead to precariousness, poverty and instability, leading to social inequalities in health and leading to new problems for different social groups (migrants, unemployed, prisoners, young people, the elderly, women, etc.)The fight against the social inequalities in health is a public health priority in France. The community approach is particularly appropriate to reach and support vulnerable populations. In occupational therapy, we can intervene with this precarious public by considering the issues in terms of occupation. Henneke van Bruggen (2014) suggests to "engage in occupational justice by developing community-based approaches based on occupation in which all individuals find their place. " The community approach is based on a specific methodology in several phases. The first one focus on the community that needs to be identified and recognized. The second phase corresponds to the community diagnosis that highlights the occupational and quality of life problems of the community. Then it is the planning of the actions, and their realization. Finally come the evaluation of these actions. This approach is a bottom-up approach, which starts from the community to co-build actions. The occupational therapist must be committed to fostering the well-being of precarious communities. He acts as an agent of change. The occupational therapist must be ready to learn from communities, to know communities, their context and their habits to act differently in practice.
